H1N1 VirusFVHD is working with the Connecticut Department of Public Health to vaccinate all residents within our district. FVHD hopes to voluntarily vaccinate all residents of Avon, Barkhamsted, Canton, Colebrook, East Granby, Farmington, Granby, Hartland, New Hartford & Simsbury.

We urge pregnant women and women that may be pregnant to see their primary healthcare provider for the vaccination. We also urge parents of children that have underlying health conditions to see the child's healthcare provider for the vaccine. If any resident is unable to receive the vaccination from their primary health care provider FVHD will provide the vaccine free of charge at public clinics throughout the district's towns.

1. Appointments are necessary. Please call our hotline at (860) 269-6214 to schedule an appointment. No walk-ins will be accepted.

2. To expedite the process on the day of the clinic, you may download and print the Flu Vaccine Consent Form. This form must be completed and signed prior to receiving a vaccine.
